Allied health plan participants can
choose from a variety of Preferred Provider Organizations in
their state. To find out more on a specific PPO
network, click on one of the links below. The links allow you to lookup specific providers or create a network
provider directory in your area.

insured member and you're unsure of your PPO network, the
information is on your insurance ID card. Click
