Members of this Community Health Plan (CHP) are covered under Allied National’s Freedom Plan – a major medical option – and are free to go to any provider for services. However, if a member chooses a Community Health Plan provider, they will have certain enhanced benefits including a LOWER deductible. CHPs are contracted with your plan and as such have agreed to accept your plan.
If you decide to use a non-CHP provider, reimbursement to your health care providers will be based on a specified percentage above Medicare levels.
Community Health Plan members have the support of Allied’s Elite Experience Team to help you with balance bills or any questions a provider might have.
Register for your Self-Service Account and gain access to your Summary Plan Description; and request service on issues ranging from changes in coverage to address updates. View a list of CHP providers and click “Find a provider”. You will need a username and password which may be provided to you by either your employer or agent.
