You have the Freedom Hybrid Plan. This plan allows you to choose the provider you want without restrictions or penalties. You can see a physician in your Preferred Provider Organization (PPO) network or go out of network – you will never be penalized for seeing an out-of-network provider. You may visit any health care facility without any restrictions - there's no required network for hospitals or other facilities.

Your Elite Experience team can help you if you receive a balance bill from a provider. Our team will work with providers to make sure you pay nothing more than your copays, deductible and coinsurance cost sharing.
Enrollment in the High-Cost Drug Program is mandatory for all eligible members to receive coverage for High-Cost Drugs (any tiers) over $500 per fill and for all specialty drugs (tier 4). Check your Summary plan description for more information and eligibility rules.
