Office visits and rich outpatient benefits, plus fixed cash payments for surgery and hospitalization

Your Cost Saver plan covers all outpatient services after your deductible and a simple copay for each service. It includes coverage for generic prescription drugs. Surgery facility and inpatient admissions are covered under a fixed indemnity payment of a specific dollar amount for each surgery or inpatient confinement subject to certain limitations.

Your plan also covers eligible charges for all preventive care services considered necessary by the Affordable Care Act, such as immunizations, mammograms and pap smears.

Cost Saver is a PPO network plan. Check your ID Card for your network and look for providers here.

Using a PPO provider will assure you of the best benefits and discounts under your health plan.

• Coverage is available for spouses and dependent children up to age 26.
• Generic Drug Discount: Cost Saver covers Tier 1 generic drugs for a small copay. Check your Rx formulary on our Prescription Benefit Plan page for a list of the generics that are available. Brand drugs are available at a discount.
• Your Cost Saver plan features immediate access by phone or computer to a physician through your telehealth & wellness benefit.
