Benefits That Help You Save Money
  1. Register for your Self-Service Account and gain access to your Summary Plan Description; and request service on issues ranging from changes in coverage to address updates. For your convenience, an electronic (PDF) version of your ID card also is available online on your Self-Service Account. Click HERE to learn how to access your Self-Service Site for the first time.
  2. Find a Provider. If you want to use a physician in your PPO, this link will help you view your options.
  3. Show your ID card to your provider at the time of service!
Your plan pays 100% preventive care benefits with no deductible.
Members can choose any dentist for services without a penalty. However, the Aetna Dental PPO is automatically included (except in Texas and Virginia). Voluntary use of its dentists may help reduce coinsurance costs.

Vision Care:

If you have vision care as part of your Dental Design plan, your plan is administered by National Vision Administrators (NVA). You'll get the best discounts on many of their services or materials by using the NVA PPO Network. To find a physician in your PPO, the Find a Provider link will help you view your options.
