1: Enter Your NPI
2: Enter Your Name, Phone Number and Email
​3: Click on the Welcome Screen to Set Up Your Profile
  • Create a password and complete your profile or wait until your welcome email arrives

3 Things to Do Now After Registering
to Complete Your Profile & Membership

​Congratulations! You've registered for National Direct Primary Care Physicians. Here are the next three things you need to do now to complete your membership.
Step 1: Complete Your Profile
  • Edit your profile with:
    • Name and contact information
    • Physician profile information to show on website
    • Plan options
    • Choose membership options
    • Enter bank account information to manage your patient transactions

  • Business Associate Agreement
Step 2: Watch for a "Complete Your Application" Email
  • Click on the email link to complete your credentialing
    • Information requested includes medical school information
  • Once completed, click on submit
Step 3: Watch for Your Affiliation Email
  • Once finalized, you'll be affiliated with National Direct Primary Care Physicians and potential patients will be able to choose you as their DPC physician.
